The contact form collects the information you submit, including name, phone number, email address, state, area of interest, preferred response method, and message. Hosting and security systems may process IP address, browser, device, page, timestamp, and security-event data.
Do not submit sensitive information
Do not submit a Medicare number, Social Security number, medical records, detailed health information, banking information, payment information, or insurance application information through the website form or ordinary email. The form is intended only to request general contact.
How information is used
Information is used to respond to the request, schedule or prepare for a consultation, operate and secure the website, maintain appropriate business records, and comply with applicable obligations. If you later apply for coverage, separate carrier, enrollment platform, and privacy notices may apply.
Sharing and sale
Contact information is not sold for money. Information is not shared with another third-party marketing organization for Medicare marketing or enrollment without the prior express written consent required by applicable law. Service providers supporting hosting, email, scheduling, security, and recordkeeping may process information as needed to provide those services. Cookies and security
The site uses an essential session cookie for form security. It does not currently use advertising pixels or cross-site behavioral analytics. The form uses a one-way hashed network and browser identifier to limit repeated submissions. Rate-limit data is designed to expire after one hour and does not contain form contents. Diagnostic logs record privacy-safe status codes and timestamps, not form contents or credentials.
Retention
Information is retained only as reasonably needed to respond, maintain appropriate records, prevent abuse, resolve disputes, and comply with legal or contractual obligations. No internet system is completely secure.
Your choices
